· Click into your new form and scroll down to Integrate
· Scroll down until you see QR code
· Click Generate image at bottom of page
· Ensure that all fields are filled out then click Generate image
· Download
· (Optional) save image or copy URL from download
· Save
· Print
· Scan with Smartphone
